About the Diabetic Education Center

The Yoakum Community Hospital Diabetic Education Center consists of a registered dietitian and a registered nurse with a special cerfification in diabetes education.  They provide one-on-one nutrition and lifestyle consultations to patients, as well as group education and support meetings.  At YCH, we work to build a support network to help you develop a positive lifestyle and protect your future health.

Our Two-Step Approach to Diabetes Education

1.  Self-Management Training
A certified diabetes educator helps patients learn more about the pathophysiology of their disease and how to recognize and prevent serious complications. Areas of focus include:
  • Disease Process
  • Healthy eating and meal planning
  • Monitoring blood glucose
  • Medications
  • Exercise
  • Prevention of disease complications
2.  Medical Nutrition Therapy
A registered and licensed dietitian helps patients learn about the interaction of different foods with their disease process.  Arears of focus include:
  • Individual diet analysis and planning
  • Portion planning with food models
  • Diet stategies (carb counting, exchange system, etc.)
  • Dining out strategies
  • Low blood sugar prevention and treatment
  • Effects of exercise on meal planning
There is a fee for services at the YCH Diabetic Education Center, however, most insurance carriers, including Medicare, will cover the cost.

How the Diabetic Education Center Helps

By participating in our program, individuals diagnosed with diabetes learn valuable disease management solutions in a caring, supportive environment.  Through an innovative, two-step approach, we help patients gain confidence in their ability to manage their diabetes and help them prevent future health issues.
