Your Medical Records
Copies of your own medical records, those of a minor child, or the medical records of someone for whom you are the legal guardian or hold a Durable Power of Attorney for Healthcare may be requested. Proof of legal guardianship or a Durable Power of Attorney for Healthcare is required for access of these records. To obtain copies of your medical record, please complete the Authorization for Disclosure/Inspection of Protected Health Information form. Download PDF.
Completed forms can be delivered to the Yoakum Community Hospital Health Information Department, faxed to (361) 293-2167 or you can mail the completed form to the following address:

Once your request has been processed, you may pick-up the copies of your records either in person or they can be mailed to your home address. If you choose to pick-up your records in person, you will be required to present photo identification. You may send a representative to obtain your records only if they are listed on the authorization form under the section entitled “Released To”. It is required that your representative provide a copy of your photo identification as well. Special arrangements can be made if you request records be sent via fax or email. Please reach out to us at the number below. Please be aware that there may be a charge for your records. If a fee applies, we will notify you before sending the records.

For a copy of your X-Ray, CT, MRI, or Mammogram images, please contact the Radiology Department at (361) 293-2321 ext. 373. Completion of an authorization form, photo ID or legal documents as above may still be required.
For a copy of your Bill / Itemized Statement, please contact the Business Office at (361) 293-2321 ext. 301
