We are excited to announce that Yoakum Family Practice now offers Telemedicine visits!
Please note that not all visits will meet criteria for Telemedicine. Registration will inform you if your visit meets criteria. During your Telemedicine visit, the provider may determine a face-to-face visit is needed. If needed, we will schedule you an appointment to come in.
Process
- Each individual patient will require an email address to use the Telemedicine platform. If you do not have a separate email address, you will need to create one before scheduling your appointment.
- Please be prepared to provide your primary contact number that the provider will be able to reach you on at the time of your appointment.
- After you schedule your appointment you will receive 3 emails fromJoinCareTeam. The 1st email will be to confirm your account – please click the link in the email to confirm. The 2nd email will provide your username and password to login to complete your registration – complete your registration online (Registration cannot be completed on the smartphone app). The 3rd email will confirm your appointment date/time with the provider. Please follow the steps in each email accordingly!You can join your appointment by a camera enabled laptop/computer by going to the website https://yoakumtelehealth.joincareteam.com (Only runs in Chrome and Firefox). For smart phone users: Download the UcareConnect app. (There are two apps in the app store –DO NOT pick the one that says Provider).
